Insurance and Fees Policy
Kristen maintains a smaller practice to ensure that a high level of care coordination is provided to all of her patients. Concierge services are provided through a non-retainer and fee-for-service model. Kristen provides additional services between appointments, as needed (see below: other professional services).
Please note that because Kristen is a fee-for-service provider, she is not contracted with any insurance companies. A superbill can be provided for possible out-of-network reimbursement from the your insurance company. You can submit your superbill directly to your insurance company by using Reimbursify. Please consult with your insurance company for any questions about your plan prior to your visit.
